WHY THE INGREDIENT WORKS
From swallowed tablet to biological effect
Sildenafil: Sexual stimulation triggers nitric oxide release, which increases cGMP in erectile tissue. PDE5 normally breaks cGMP down. Inhibiting PDE5 allows the cGMP signal to persist longer. Sildenafil is mainly metabolized through CYP3A4, with CYP2C9 contributing. Tadalafil: Nitric oxide increases cGMP during sexual stimulation. PDE5 degrades cGMP. Tadalafil inhibits PDE5 and has a longer pharmacologic duration than several other drugs in the same class. It is metabolized mainly through CYP3A4.

HOW TO THINK ABOUT "WHAT SHOULD I TAKE?"
The right dose is patient-specific
A high-fat meal can delay absorption of conventional sildenafil tablets. Formulation can matter, so follow the directions supplied with the exact product. Conventional tadalafil tablets can generally be taken without regard to meals. Follow the exact instructions for a compounded or alternate dosage form.
Use the dose and frequency on your own prescription label. Strengths exist because age, health conditions, other medicines, effectiveness, and side effects can change what is appropriate. Do not copy another person's dose or increase a dose because the first attempt did not work as expected.
